Job summary

One Senior Care is seeking a Physical Therapist to provide accurate, evidence-based therapy to participants across centers, homes, and healthcare facilities. You will perform comprehensive evaluations, design individualized treatment plans, and help participants improve mobility and independence within a structured care model.

Qualifications

• Must hold a current PT license within the practicing state.
• Experience working with frail or elderly population is a plus; hands-on training provided.
• Demonstrate clinical competency with geriatric population.

Responsibilities

• Conduct comprehensive physical therapy assessments and ongoing evaluations.
• Review medical history and establish individualized plans of care and treatment goals.
• Provide therapeutic exercise, activities, gait training, and physical modalities.
• Document evaluations, treatments, and progress in the EMR.
• Supervise Physical Therapist Assistants as required.
• Deliver therapy in multiple settings including the center, homes, facilities, and hospitals.
