Job summary

Endeavor Health in Gurnee, IL is seeking a Staff Physical Therapist II for outpatient services. This full-time role (36–40 hours) offers clinical evaluation, treatment, and follow-up for physician-referred patients in a clinic open Mon–Fri 8am–7pm.

You will document evaluations, progress notes, discharge summaries, and billing; participate in supervision of students; require Doctorate in Physical Therapy, IDFPR licensure or eligibility, and BLS/CPR certification.

Qualifications

• Outpatient orthopedics experience required or preferred.
• BLS/CPR certification required.
• Vestibular or pelvic health interest is a plus.

Responsibilities

• Provide evaluation, treatment and follow-up services to physician referred patients.
• Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries and billing.
• Participate in the clinical supervision of students.

Hourly Pay Range:

$38.67 - $59.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Location: 7900 Rollins Rd. Gurnee, IL
• Full Time/Part Time: Full time 36 - 40 hours
• Hours: Clinic open M-F 8am-7pm, two evenings/week required
• Required Travel: No

What you will do:

• Provide evaluation, treatment and follow-up services to physician referred patients

• Participates in the clinical supervision of students

What you will need:

• License: Current licensure or eligible for licensure as a Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR) required
• Education: Doctorate in Physical Therapy
• Certification: BLS/CPR certification from American Heart Association required
• Experience: outpatient orthopedics
• Skills: Outpatient orthopedics with interest in vestibular or pelvic health
