Job Description

  • This is an in-house registry position at Lake Forest Hospital. Minimum shift requirements include 2 weekend shifts and 4 weekday hours per month. Holiday rotation required.

The Physical Therapist reflects the mission, vision, and values of NM, adhering to our Code of Ethics and Compliance Program, and complies with all relevant policies and standards.

  • A licensed physical therapist in Illinois, adhering to the Code of Ethics/Standards of Practice.
  • Includes direct patient care, teaching, committee participation, and process improvement.
  • Serves as a role model in professional, clinical, communication, education, and leadership activities.

Responsibilities:

  • Demonstrate NM values: teamwork, patients first, integrity, and excellence.
  • Maintain a friendly, professional demeanor and appearance.
  • Provide accurate, timely information to patients and address questions or concerns.
  • Use effective service recovery skills for problem-solving.
  • Manage patient care plans collaboratively with PT/PTA teams.
  • Show teamwork and effective communication across departments.
  • Use resources efficiently and manage work schedules effectively.
  • Participate in quality improvement, cost reduction, and service growth initiatives.
  • Apply clinical skills appropriately, perform re-evaluations, and adjust treatment plans.
  • Take responsibility for patient management from initial contact to discharge, ensuring safety and progress.
  • Document all notes accurately in EMR, reflecting patient response and progress.
  • Provide appropriate educational materials to patients.
  • Participate in departmental committees, projects, and educational efforts.
  • Build professional relationships with healthcare team members.
  • Foster respectful communication and accept constructive feedback.
  • Complete non-patient care responsibilities timely, including documentation and scheduling.
  • Mentor students and participate in clinical programs.
  • Maintain productivity standards and be flexible to meet patient care needs.

Qualifications:

  • Required: Bachelor’s Degree in Physical Therapy, Illinois licensure, BLS certification.
  • Preferred: Master’s or Doctoral Degree in Physical Therapy.
