Job Description

This is an in-house registry position based at Lake Forest Hospital. The minimum shift requirements include 2 weekend shifts per month, 4 weekday hours per month, and participation in a holiday rotation.

The Physical Therapist role aligns with NM's mission, vision, and values, adhering to our Code of Ethics and Compliance Program, and complying with all relevant policies and standards.

  • Licensed physical therapist in Illinois, adhering to the professional Code of Ethics and Standards of Practice.
  • Responsibilities include direct patient care, teaching, committee participation, and process improvement.
  • Serve as a role model for therapists and interns, demonstrating professionalism, clinical skills, communication, and leadership.
Responsibilities
  • Demonstrate NM values: teamwork, patients first, integrity, and excellence.
  • Maintain a friendly, professional demeanor and appearance.
  • Provide accurate information and timely updates to patients and stakeholders.
  • Use effective service recovery skills to address issues.
  • Manage patient care plans collaboratively with PT/PTA teams.
  • Show teamwork, communicate effectively, and respect diverse opinions.
  • Utilize resources efficiently and manage work schedules effectively.
  • Participate in quality improvement, cost reduction, and service enhancement initiatives.
  • Apply clinical expertise in evaluations, treatments, and re-evaluations, documenting appropriately.
  • Assume full responsibility for patient management from initial contact to discharge.
  • Adhere to documentation standards in the EMR, including daily notes and discharge summaries.
  • Educate patients using appropriate materials and methods.
  • Engage in departmental committees, projects, and educational efforts.
  • Develop professional relationships within healthcare teams and participate in marketing efforts.
  • Promote respectful communication and accept constructive feedback.
  • Complete non-patient care tasks timely, including scheduling and administrative responsibilities.
  • Mentor students and participate in clinical programs.
  • Maintain productivity and flexibility to meet patient and department needs.
Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in Illinois.
  • BLS certification from the American Heart Association upon hire.

Preferred:

  • Master’s or Doctoral Degree in Physical Therapy.
