Legacy Healthcare Services is seeking a Physical Therapist in Florida to deliver individualized rehabilitative care within senior living communities. You will work on a private treatment basis, plan programs, and collaborate in clinical conferences to coordinate plans of care.

The role emphasizes one-on-one sessions, a patient-centered approach, and opportunities to grow professionally within a not-for-profit provider serving hundreds of communities across the country.

Qualifications

• Degree in area of discipline is required.
• Current Physical Therapy license in state of practice, or ability to obtain before starting.
• Excellent interpersonal, fiscal, and clinical skills.
• Basic computer proficiency is required.

Responsibilities

• Provide rehabilitative services to patients under medical orders as part of a team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate care plans.
• Treat residents one-on-one in private sessions with functional-based approach.
• Customize treatment sessions to meet individual patient needs.
