Corewell Health is seeking a Physical Therapist for home care in Michigan, serving St Joseph and Benton Harbor. The role includes evaluating patients, designing and adjusting treatment plans, and educating patients and staff. The position offers incentives up to $15,000 and a standard full-time schedule.

You're required to hold a Michigan PT license upon hire, plus related BLS certifications. A master's degree is preferred and experience is advantageous.

Qualifications

• Bachelor's Degree or equivalent from an accredited physical therapy program.
• Physical Therapist license in Michigan required upon hire.
• Driver's license valid in Michigan for Home Care upon hire.
• Basic Life Support (BLS) certification current; American Heart Association preferred.
• Basic Life Support (BLS) certification current; American Red Cross required.
• Master's Degree preferred.
• 6 months of relevant experience preferred.

Responsibilities

• Performs evaluation procedures to assess disability level and plan treatment.
• Provides treatment program to restore maximal function and adjust as needed.
• Documents evaluation and treatment per department standards and reimbursement rules.
• Instructs and trains rehab staff and students regarding patient needs.
• Participates in process improvement, patient satisfaction, and program development.
• Demonstrates productivity and pursues continuing education and new skills.

This position works in St Joseph and Benton Harbor.

Job Summary

Provides therapy to patients based on patient need and medical direction. Provides patient/caregiver education and arranges for adaptive equipment as needed.

Essential Functions

• Performs evaluation procedures to assess the level of disability with regard to scope of practice, goals and a plan of treatment, and to make appropriate recommendations.
• Provides a treatment program and education to restore and return maximal function to the patient. Assesses effectiveness of treatment and modifies goals / treatment methods, as needed to meet changing needs.
• Documents evaluation and treatment of each patient in compliance with department standards, medical-legal and reimbursement procedures.
• Instructs and trains other rehabilitation staff, nursing, other medical personnel and students regarding patient needs and rehabilitation services.
• Participates in ongoing process improvement, patient satisfaction initiatives/improvement, program development and marketing activities.
• Demonstrates productivity as indicated by department standard.
• Improves skill areas as demonstrated by applying continuing education in-services, participating in competencies in applicable areas, and developing new concepts of patient care and additional skills with various population groups.

Qualifications

Required

• Bachelor's Degree or equivalent from an accredited physical therapy program
• Physical Therapist - State of Michigan Upon Hire required
• Driver's License - State of Michigan Home Care Business Upon Hire
• Basic Life Support (BLS) - American Heart Association 90 Days Or
• Basic Life Support (BLS) - American Red Cross 90 Days required
