Qualifications

• Active MO PT license required.
• Home health experience preferred.

Responsibilities

• Provides PT assigned services to patients in the home setting under physician plan of care and supervision of a licensed PT.
• Participates in communication and case conference activities to promote continuity and quality of patient care.
• Maintains compliance with agency codes of ethics and standards.
• Adheres to state and Federal regulations, licenses, payer requirements, and safety guidelines.
