Responsibilities

• Provides PT services to patients in their home under physician plan of care.
• Communicates with care team to ensure continuity and quality of patient care.
• Follows agency protocols and maintains clinical standards.
• Complies with agency ethics, policies, and regulations.
• Maintains safety and infection control practices during visits.
• Performs miscellaneous duties as assigned by Supervisor.
