Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time opportunity performing home health visits for our office located in Baltimore, MD. This office services adult and geriatric clients on a per-visit basis in territories throughout the White Marsh area.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise the care plan regularly, collaborating with nursing staff as part of the multidisciplinary team.
5. Educate clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. Current MD Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; prior experience with electronic medical records (EMR) preferred.
