Job Description
The Maryland Primary Care Program (MDPCP) Care Coordinator will work closely with Primary Care providers in Maryland to identify, screen, track, monitor, and coordinate patient care, especially for those with multiple chronic conditions. The role involves developing and delivering comprehensive primary care, collaborating with interdisciplinary teams, and supporting patient-centered care initiatives. Responsibilities include meeting performance metrics, conducting outreach, providing wellness visits and transitional care management, educating patients, coordinating referrals, and participating in care team meetings.
Qualifications
- Understanding of MDPCP program, mission, vision, and goals
- Strong computer and EHR skills
- Experience with Medicare aged patients and value-based programs (preferred)
- Experience in outpatient medical settings
- Ability to travel for patient visits
- Compliance with HIPAA regulations
Salary and Benefits
The salary range is $51,000 - $63,000 annually, excluding bonuses and benefits such as medical, dental, vision, life insurance, 401K, paid time off, and wellness programs. An annual bonus targeted at 10% may be available, with the actual base pay determined by experience, education, and location.
Technical Requirements
Remote workers must have a minimum internet speed of 5 MBPS download and 3 MBPS upload, verified via speed tests. Employees working from home are eligible for expense reimbursement for internet costs.
