BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a PRN, part-time, or full-time opportunity performing home health visits throughout Schuylkill County.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home with appropriate medical support.

Responsibilities for a PT include:

  1. Make home visits to clients in designated geographic areas.
  2. Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
  3. Implement protective interventions to prevent wound formation and promote healing, utilizing evidence-based dressings and topical agents.
  4. Develop and update individualized care plans, including manual exercises, gait training, balance, and other interventions.
  5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
  6. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
  7. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications include:

  • A current Pennsylvania Physical Therapist (PT) license.
  • Graduation from an APTA-approved program or accredited by the Committee on Allied Health Education and Accreditation.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
