Job summary

Careage Home Health - King County is seeking a Physical Therapist to join our home health team in Washington. You will provide one-on-one care across multiple locations in Rural South King County, including Kent, Auburn, Federal Way, Des Moines, and Covington.

We offer a competitive hourly rate with travel time, a flexible schedule with no on-call and no weekends, mileage reimbursement, and opportunities for professional growth through comprehensive training and advancement potential.

Qualifications

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association (APTA).
• Currently licensed as a physical therapist in the state of Washington.
• Two (2) years' experience as a physical therapist preferred.
• Home Health experience preferred.
• Detail oriented, self motivated and capable of efficient time management.
• Critical thinker capable of working collaboratively with a team.
• Valid WA State driver's license.

Responsibilities

• Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
• Work with other home care personnel in patient care planning.
• Direct and supervise Physical Therapy Assistant and Home Health Aide as assigned.
• Take initial patient history and perform initial evaluation.
• Consult with physicians regarding changes in treatment.
• Instruct patients and family in home programs and activities of daily living.
• Participate in Quality Assessment and Performance Improvement activities as assigned.
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes in condition.
• Appropriately utilize ICD-10 codes.
