Job summary

Careage Home Health is seeking a licensed Physical Therapist to join our Seattle-area team. The role involves one-on-one home visits, developing plans of care, and supervising PT aides across multiple Greater Seattle locations.

The position emphasizes flexible scheduling with no on-call or weekend work, a competitive hourly rate, and travel time reimbursement. Washington licensure and a valid driver's license are required.

Qualifications

Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
Currently licensed as a physical therapist in the state of Washington.
Two years of experience as a physical therapist preferred.
Home Health experience preferred.
Detail oriented, self motivated and time management focused.
Critical thinker capable of working collaboratively with a team.
Valid WA State driver's license.

Responsibilities

Provide physician orders and plan of care under the direction of the Director of Clinical Services.
Collaborate with home care personnel in patient care planning.
Direct and supervise Physical Therapy Assistant and Home Health Aide as assigned.
Take initial patient history and perform initial evaluation.
Consult with physicians regarding treatment changes.
Instruct patients and families in home programs and activities of daily living.
Participate in Quality Assessment and Performance Improvement activities.
Complete and submit OASIS assessments and related documentation.
Use ICD-10 codes appropriately.
