Job summary

BAYADA Home Health Care is seeking a Physical Therapist for per diem, part-time work in Colorado. You’ll provide home health PT services, focusing on mobility, gait, and functional independence within a supportive team.

Applicants should hold a Colorado PT license (or be eligible) and have experience with EMR systems. This role offers autonomy, flexible scheduling, and meaningful patient relationships in a mission-driven organization.

Qualifications

• Current Colorado PT license (or eligibility)
• Graduation from an accredited PT program
• Comfort with EMR systems (training provided)

Responsibilities

• Perform mobility, strength, balance, and safety assessments
• Develop individualized, evidence-based treatment plans
• Provide gait training, manual therapy, balance work, and therapeutic exercise
• Implement skin and wound-prevention strategies
• Educate clients and families to promote long-term success
• Document care using our EMR system

Education

Compensation & Schedule

• Competitive weekly pay

• Pay: $65-70 per point, based on qualifications
