Position: Physical Therapist ‐ Home Health (PT ‐ HH)

Start Date: ASAP

Available Shifts: 8 D

Pay Rate: $2174.27 ‐ $2382.44

Contract Details

1. Approval: JUNE ARMBRUSTER PT DOO will approve the contractor's time each week.
2. iCIMS Req #: 2025‐33961
3. Duration: 13 weeks with an option to extend
4. Schedule: Monday to Friday, 8 am to 5 pm
5. Weekend Rotation: 1 Saturday per month if needed for admit
6. On Call: None
7. Region & Setting: Home Health in the South West territory
8. Territory: Algonquin, Huntley, Lake in the Hills, Elgin, South Elgin, St Charles, Pingree Grove, Marengo, Carpentersville, Bartlett, Schaumburg, and surrounding areas
9. Workload Justification: Workload demand
10. Reporting Location: 765 ELA RD, Lake Zurich, IL 60047, Suite 105
11. Travel: Approximately 200-300+ miles per week
12. Productivity Expectations: 30 points weekly
13. EMR System: HCHB
14. Experience Required: Home health experience preferred
