Job summary

Mercyhealth Visiting Nurses Association, Inc. seeks a licensed Physical Therapist for home health in northern IL and southern WI. The role includes evaluating patients, developing and implementing PT plans, and coordinating with physicians and staff.

You will perform OASIS assessments and ensure safe, effective care. Mandatory licensure in WI or IL and CPR/BLS within 90 days of hire; home health experience preferred.

Qualifications

  • Graduate of an accredited PT program (Bachelor, Master, or Doctorate).
  • Wisconsin and/or Illinois PT license as required.
  • CPR or BLS certification within 90 days of hire.

Responsibilities

  • Reviews referrals and patient data to determine PT treatment plan.
  • Tests and measures strength, ROM, balance, gait, and functional capacity.
  • Plans written treatment plan based on evaluation.
  • Administers manual exercises and therapeutic activities.
  • Educates patient and family on home exercise and safe device use.
  • Documents treatment, response, and progress; updates chart or EHR.
  • Contributes to OASIS start of care, recertification, and discharge assessments.
  • Collaborates with physicians and teammates to integrate PT care.

Job description

Job Duties
  • Reviews Physician's referral, patient's condition and medical records to determine physical therapy treatment required.
  • Obtains history and subjective components of the evaluation.
  • Tests and measures patient's strength, balance, range of motion, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise treatment programs.
  • Observes posture, position, and palpates anatomical structures.
  • Plans and prepares written treatment plan based on evaluation of patient data.
  • Administers manual exercises to improve and maintain function.
  • Instructs, motivates, and assists patient to perform various physical activities and in use of assistant and supportive devices such as crutches, canes, and prostheses.
  • Administers treatments involving application of physical agents.
  • Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit.
  • Administers massage, soft tissue mobilization, joint mobilization.
  • Administers traction to relieve pain.
  • Records treatment, response, and progress in patient's chart or enters information into computer.
  • Instructs patient and family in treatment procedures to be continued at home.
  • Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate physical therapy treatment with other aspects of patient's health care.
  • Competent in completing home health therapy start of care, recertification, resumption of care, transfer, and discharge OASIS assessments.
  • Assists with the supervisory visits of HHA as required by regulations.
  • May assist with orientation and training of new partners as needed.
Requirements
  • Graduate of an accredited Bachelor, Master or Doctorate program for Physical Therapy.
  • Home Health experience is preferred.
  • License to practice Physical Therapy in the state of Wisconsin and/or Illinois as required.
  • Approved CPR or BLS certification required within 90 days of hire.
  • Passing the Driver’s License Check and/or Credit Check (for those positions requiring).
  • Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
  • Must be able to follow written/oral instructions.

PAY RANGE: $42.84 - $64.26
