Job summary

Dartmouth Health Home Care is hiring a Physical Therapist to join our caring team. The PT will screen, evaluate, and treat patients in home health settings, coordinating care with an interdisciplinary team and documenting progress.

Qualified candidates are licensed in VT or NH (or compact) with an accredited PT degree, and a valid driver’s license. Prior home health experience is preferred; relocation and CEUs are available as benefits.

Qualifications

• Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school in Physical Therapy.

Responsibilities

• Screen, evaluate, re-evaluate and treat patients.

Responsibilities

Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

The PT will:

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.

• Communicate with all members of the health care team.
• Document evaluations, treatment goals, and objectives.
• Teach patients and family members the rationale for treatments and procedures.

Qualifications

REQUIRED QUALIFICATIONS

• Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school in Physical Therapy

PREFERRED QUALIFICATIONS

• One year of experience as a PT in a home health care setting is preferred

Required Licensure/Certifications

• Must hold at least one active unencumbered PT license in either VT or NH at the time of hire. Employees must have active unencumbered PT licenses in both VT and NH within 30 days of their hire date.
• Valid driver's license with a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire

Salary ranges within each pay grade are based on qualifications, skills, experience, equity in comparison to similar positions, and external market values.
