Job summary

Dartmouth Health Home Care is seeking qualified Physical Therapists for full-time and part-time (20-24 hrs) roles in the Upper Valley area. The therapist will screen, evaluate, re-evaluate, and treat patients and coordinate with the interdisciplinary team.

Qualifications

• Must be licensed Physical Therapist in VT and NH or hold a compact license.
• Graduate of an accredited Physical Therapy program.

• Communicate with health-care team members.
• Document evaluations, goals, and progress.
• Educate patients and families on treatments.
