Job Description

Position Overview

As a Home Health Physical Therapist, You Will:

• Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and disabilities to attain highest physical function.
• Test and screen patients' physical strengths to assist in evaluating their level of function and record findings to develop or pursue treatment programs with measurable objectives.
• Develop and implement conditioning and rehabilitation programs aligned with the physician’s Plan of Treatment and overall patient goals. Adjust treatment as needed.
• Consult with physicians and clinical team members to refine treatment plans and develop new strategies.
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides, providing written instructions and monitoring documentation completion.
• Document patient care observations, interventions, and evaluations thoroughly and promptly, ensuring compliance with orders and policies.
• Report patient progress to physicians and team members, submitting evaluations, treatment plans, progress reports, and discharge summaries.

Required Experience/Skills

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Additional Details

Scheduled Weekly Hours: 40

Pay Range: $85,400 - $117,500 per year

Location: Washington, North Carolina
