Therapy Partner Solutions in Deland, FL is seeking a Physical Therapist to join our Home Health team on a full-time, per-visit rate basis of $60–$120 per hour.

You will work with a diverse caseload, supported by a collaborative, multidisciplinary team, and have opportunities for growth through mentorship, advanced training, and certifications.

Qualifications

• Graduate from an accredited physical therapy program.
• Active or pending state physical therapist license.
• Comfort working independently in the home setting.

Responsibilities

• Provide physical therapy services in patients' homes.
• Collaborate with a multidisciplinary Accent Care Home Health team.
• Document care plans and progress for each patient.
• Engage in professional development and certifications.

License & Experience

Requirements for this position include the following:

• Graduate (current or pending) from an accredited physical therapy program

• Active or pending state physical therapist license

• Comfort working independently in the home setting
