Physical Therapist Care Manager | Central Florida Home Care

Schedule: Full Time, 40 hours per week

Shift: Days

Location: Team Green: This position will be conducting patient home visits in the Kissimmee, Celebration, St. Cloud, and Osceola County area.

The role you’ll contribute: The Home Health Physical Therapist (PT) Care manager is a professional therapist who coordinates and directs the home care patient’s services based on individual patient needs. The PT Care manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching, and decision-making skills. The PT is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience are consistently applied to new patient populations. The PT Care manager cares for a caseload of home health patients requiring therapy as the primary service by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.

1. Coordinates and directs the care of a caseload of home patients requiring physical therapy as the primary skill.
2. Sets priorities of home care caseload adapting to the changing needs of the home care patients and families.
3. Performs an evaluation, assessing function using a method which objectively measures activities of daily living.
4. Utilizing assessment data, formulates a patient-specific plan of care along with the patient, family, and physician.
5. Periodically reassess the patient every 30 days and document the measurements in the clinical record.
6. Observes and records activities and findings in the clinical record and reports to the physician the patient’s reaction to treatment.

The expertise and experiences you’ll need to succeed:

1. Valid Driver's License
2. Physical Therapist License
3. CPR Certification
4. Minimum of one-year relevant clinical physical therapy experience
5. Graduated from an approved physical therapy curriculum as required by the state which issued the license

Preferred qualifications:

1. Recent, relevant experience in a Medicare-certified home health agency as a care-manager
2. Master's or Doctorate in Physical Therapy
3. Home Health Care-Manager Certification or COS-C
4. Working knowledge of community resources and the ability to refer patients and families appropriately
