Job summary

Dignity Health Woodland Memorial Hospital in Woodland, CA is seeking a Physical Therapist to help patients rebuild or maintain their physical wellbeing. You will diagnose, evaluate and treat patients, developing individualized plans of care to improve function and quality of life.

You will conduct initial evaluations, develop plans of care, provide treatments, and collaborate with the care team while maintaining productivity standards. A $3,000 sign-on bonus is offered to eligible new hires.

Qualifications

• High School Diploma required; 2 years experience in health care.
• CA licensure for Physical Therapist required.
• BLS/CPR certification required.
• Driver's license with CA and automobile insurance.
• Auto insurance is required.

Responsibilities

• Perform initial evaluations and develop plans of care.
• Provide direct treatments and monitor patient progress.
• Meet productivity standards and document care per guidelines.

Job Summary and Responsibilities

As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.

Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.

To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.

• Meets the discipline-specific productivity standard.
• Demonstrates competency in the care of patients during field observation by the clinical supervisor.
• Completes and submits documentation following established guidelines as measured by a designated evaluation tool.
• Physician orders are present and written correctly.
• Documents follow up of identified problems.
• Reviews and documents clinical progress toward goals.

We are offering a $3,000 Sign-On bonus to new hires who meet the eligibility requirements.
