You really get to know your patients and become an integral part of their care team. Your average visit is 45 minutes giving you significant time to provide meaningful therapy. You are provided broad independence but are also part of a cohesive patient-care team. You have a work/life balance that can match the flexibility you need. Locations: White River Junction, VT area Lebanon, NH area Bradford, VT area Windsor, VT area Visiting Nurse and Hospice for VT & NH covers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. When you join Visiting Nurse and Hospice for VT & NH, you ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve. Physical Therapists at VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient. The PT will: Evaluate the patient's need for physical therapy and develop an appropriate treatment plan. Carry out appropriate treatment programs in accordance with agency policies and professional standards of care. Communicate with all members of the health care team. Document evaluations, treatment goals, and objectives. Teach patients and family members the rationale for treatments and procedures. Minimum Qualifications: BS in Physical Therapy Must be a licensed Physical Therapist in VT and NH, or hold an unencumbered compact license. One year of experience as a PT in a home healthcare setting
