Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patients' care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate patients' functional status and therapy needs; assist physicians in developing the care plan.
• Observe and report patient responses and condition changes to the supervising nurse or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Instruct and inform healthcare team members, patients, and families about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the relevant state
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
