Job summary

Pinnacle Home Care in Florida is seeking a dedicated Physical Therapist to join our award-winning team providing in-home care across the Tampa area. You will perform assessments, develop treatment plans, and collaborate with physicians to improve patient outcomes.

Qualifications

• Must have a current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health preferred).
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to a reliable vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform assessments.
• Ability to walk, stand, stoop, and assist with patient handling.
• Proficiency with electronic medical records is a plus.

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation.
• Develop and implement tailored treatment plans in consultation with physicians.
• Collaborate with interdisciplinary care team to optimize outcomes.
• Provide education and support to patients and families on therapy programs.
• Maintain accurate clinical records and report changes to physicians.
• Supervise Physical Therapy Assistants per policy and state rules.
• Participate in in-services, case conferences, and team meetings.
• Identify patient needs for additional services and referrals.
• Confirm daily visit schedules for coordination with staff.
• Communicate patient progress with physicians and care teams.
• Document visits and progress notes on the day of service.

Key Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.

• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.

• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
• Proficiency with electronic medical records is a plus.
