Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a part-time or per diem opportunity performing home health visits throughout Northumberland County.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care to remain at home.

Responsibilities include:

1. Making home visits to clients in designated geographic areas.
2. Performing diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assessing and revising care plans regularly, and collaborating with nursing staff within the multidisciplinary care team.
5. Educating clients and family members on rehabilitative care and activities to promote health, safety, and independence.
6. Documenting observations, interventions, and evaluations accurately using a tablet-based system.

Qualifications:

• Current PA Physical Therapist license.
• Minimum of one year recent experience as a PT.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR systems preferred.
