As a home health Physical Therapist in our Rutland office, servicing Rutland and surrounding areas, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, allowing them to receive the medical care needed to remain at home. Our Physical Therapists deliver exceptional care to every client.
• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy care plan.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations related to client care management.

Qualifications for a BAYADA Physical Therapist:
• Current Physical Therapist (PT) license in the state
• Minimum of one year of recent work experience as a PT
• Graduation from an American Physical Therapy Association-approved program or equivalent
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
