Organization: SCAH-Sutter Care at Home - Bay
Position Overview:
Under the supervision of the Clinical Manager, the Primary Care Clinician (PCC) is an experienced physical therapist responsible for the overall case management of home health patients. This includes providing direct care with a designated visit productivity standard. The position involves planning, organizing, and directing all patient care services within the assigned caseload, ensuring quality standards are met. The PCC coordinates the interdisciplinary team to implement the patient care plan and manages visit utilization and expenses. They may also assist in agency-wide orientation, competency evaluations, and clinical consultations as requested by the Clinical Manager.
The Primary Care Clinician reports directly to the Clinical Manager and may provide functional guidance to team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs, and HHAs.
