Organization: SCAH-Sutter Care at Home - Bay

Position Overview:

Under the supervision of the Clinical Manager, the Primary Care Clinician (PCC) is an experienced physical therapist responsible for the overall case management of home health patients. This includes providing direct care with a designated visit productivity standard. The position involves planning, organizing, and directing all patient care services within the assigned caseload, ensuring quality standards are met. The PCC coordinates the interdisciplinary team to implement the patient care plan and manages visit utilization and expenses. They may also assist in agency-wide orientation, competency evaluations, and clinical consultations as requested by the Clinical Manager.

The Primary Care Clinician reports directly to the Clinical Manager and may provide functional guidance to team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs, and HHAs.

Job Description:

Education

• Bachelor's degree in Healthcare or related PT program
• Master's degree in Physical Therapy

Certification & Licensure

• PT - Physical Therapist
• BLS - Basic Life Support Healthcare Provider
• California Driver's License Class C

Experience

• One year of professional experience in an acute or similar setting required
• One year of experience in home health and/or hospice preferred
• Five years of direct patient care experience within the past five years required

Skills and Knowledge

• Thorough understanding of home care and hospice services, Medicare Conditions of Participation, accreditation requirements, QI outcomes, and infection control
• Knowledge of reimbursement patterns for Medicare and other payors
• Understanding of budget and financial reports
• Clinical competency and ongoing testing
• Self-motivated with initiative, flexibility, energy, and creativity
• Ability to build harmonious relationships with internal and external customers
• Leadership, negotiation skills, and resource management for cost-effective patient care
• Strong problem-solving and collaboration skills
