Physical Therapist - Home Health - (PT - HH)

Start Date: ASAP | Available Shifts: 8 D | Pay Rate: $2221.81 - $2434.54

The primary goal of the Physical Therapist is to restore function and minimize disability following disease or injury. They help clients reach their maximum performance within their capabilities by evaluating and treating patients using medically prescribed physical therapy programs. Responsibilities include supervising Physical Therapy Assistants, conducting initial assessments, and notifying the Clinical Team Manager of OASIS scores within established timelines. The therapist documents observations, clinical findings, problems, interventions, goals, and discharge plans. Additional duties are performed as assigned, aligned with the goals of Pathways Home Health & Hospice.

Territory Details

Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring cities as needed.
