BAYADA Home Health Care in Prescott, AZ is seeking a Physical Therapist (PT) to join our home-based care team. You will assess clients, develop individualized care plans, and guide treatment in the patient’s home.This role offers per diem scheduling, collaborative support from a multidisciplinary team, and opportunities to advance rehabilitation in a compassionate, quality-driven environment.

Qualifications

• Arizona Physical Therapist license is required.
• Graduation from an APTA-approved program or AMC-accredited program as indicated by transcript or diploma.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Perform PT assessments and develop individualized care plans.
• Deliver home health PT services and document progress.
• Implement gait training, balance, and therapeutic exercises.
• Educate patients and families on home care and safety.
• Collaborate with a multidisciplinary team and maintain EMR documentation.

FLEXIBLE SCHEDULE 1.1 PATIENT CARE SUPPORTIVE TEAM TRAINING

BAYADA Home Health Care is currently seeking a Physical Therapist (PT) to join our Prescott, AZ team. This team provides care to clients throughout Yavapai County . PT’s work with clients in their home, craft treatment plans, and apply innovative techniques to help them reach their goals. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, balance testing, and safety assessments

• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions

• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents

• Continually assess and revise the Physical Therapy - PT care plan

• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.

• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet

Compensation & Schedule:

• $75-80 pay per point model

• Flexible scheduling, M-F

Qualifications for a BAYADA Physical Therapist:

• A current Physical Therapist - PT license in Arizona

• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma

• Solid computer skills; prior experience with electronic medical records (EMR) preferred
