Job summary

Hartford HealthCare at Home is seeking a Home Health Physical Therapist to evaluate homebound patients in Plainfield and surrounding areas. You will develop individualized plans of care and deliver evidence-based treatment in the home setting, supporting mobility, safety, independence and quality of life.

Qualifications

• PT experience required.
• Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, BS degree or entry level master's degree.
• Active CT PT license.
• CPR certification required at time of start.

Responsibilities

• Evaluate homebound patients and identify functional needs.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise and other interventions to improve strength, mobility, balance, gait and daily function.
• Educate patients and caregivers on treatment plans, safe mobility and home exercises.
• Collaborate with the care team to coordinate services and monitor progress.
• Complete timely, accurate documentation in line with standards.

What You’ll Do

• Evaluate homebound patients referred for physical therapy and identify functional needs.
• Develop and implement individualized treatment plans based on each patient’s goals and condition.
• Provide therapeutic exercise and other appropriate interventions to improve strength, mobility, balance, endurance, coordination, gait, and daily function.
• Assess pain, joint mobility, motor control, posture, cardiopulmonary function, soft-tissue integrity, and the ability to complete activities of daily living.
• Educate patients and caregivers on treatment plans, safe mobility, home exercise, and strategies that support independence.
• Collaborate with the care team to coordinate services, monitor progress, and adjust the plan of care as needed.
• Complete timely, accurate documentation in accordance with clinical and organizational standards.
