Job summary

Hartford HealthCare at Home is seeking a full-time Home Health Physical Therapist to evaluate patients in their homes, develop plans of care, and deliver evidence-based treatment. You will manage a flexible schedule, focus on mobility, safety, and independence, and work with a collaborative care team across Connecticut.

You will provide one-on-one patient care, educate families, and document progress while contributing to high-quality outcomes and career growth within a supportive home health

Qualifications

• PT experience required.
• Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, BS degree or entry level master's degree.
• Active CT PT license.
• CPR certification required at time of start.

Responsibilities

• Evaluate homebound patients referred for physical therapy and identify functional needs.
• Develop and implement individualized treatment plans based on patient goals and condition.
• Provide therapeutic exercise and interventions to improve strength, mobility, balance, endurance, coordination, gait, and daily function.
• Assess pain, joint mobility, motor control, posture, cardiopulmonary function, and activities of daily living.
• Educate patients and caregivers on treatment plans, safe mobility, home exercise, and independence strategies.
• Collaborate with the care team to coordinate services, monitor progress, and adjust the plan of care.
• Complete timely, accurate documentation in accordance with clinical standards.
