Job summary

Deaconess Home Health in Jasper, IN is seeking a licensed Physical Therapist to join our in-home care team. This part-time role involves evaluating patients at home, creating treatment plans, and guiding recovery and functional movement.

You will work 3 days per week, 8-hour days, with mileage reimbursement; no after-hours on call. Requires Bachelor's in PT, active PT license in Indiana, CPR/BLS, and a valid driver's license.

Qualifications

• Bachelor’s degree from an accredited PT program (APTA approved).
• Active physical therapy license in the state you practice.
• Current CPR/BLS certification is required.
• One year of PT experience is preferred but not required.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Conduct evaluations of patients’ mobility, strength, and functional limitations.
• Implement therapy exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercises, safety, and lifestyle changes.
• Monitor progress and adjust treatment plans to achieve goals.
• Maintain accurate patient records in line with regulations.

We are seeking a skilled and compassionate Physical Therapist to join Deaconess Home Health team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.

The focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most “functional” place to make an impact on this!

Responsibilities

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.
• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.

Job Requirements

• Bachelor’s degree from an accredited school approved by the American Physical Therapy Association
• Active registered PT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year physical therapy experience in teaching or clinical setting is preferred, but not required.
• Current Drivers License
• Comply with federal, state and local laws and regulations

Pay Range

$39.18 - $58.28 /hour
