Job summary

Choice Matters Home Healthcare is seeking a Home Health Physical Therapist Assistant (PTA) to provide skilled physical therapy in Jacksonville, FL and surrounding areas. We support flexible scheduling with full-time, part-time, and PRN opportunities, backed by AI-assisted documentation and WellSky EMR.

You will work under the supervising PT to help patients regain mobility, strength, and independence, with a focus on reducing after-hours charting and improving care delivery.

Qualifications

• Current Florida PTA license required.
• Valid driver's license and reliable transportation.
• Excellent communication and organizational skills.
• Home health experience preferred but not required.

Responsibilities

• Provide skilled physical therapy services under the supervision of the Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, endurance and functional independence.
• Educate patients and caregivers on exercises, mobility techniques, and safety strategies.
• Communicate patient progress and changes with the supervising PT and care team.
• Complete accurate and timely documentation while maintaining Medicare and agency standards.

What You’ll Do

• Communicate patient progress and changes in condition with the supervising Physical Therapist and interdisciplinary care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards

• Valid driver’s license and reliable transportation
• Excellent communication and organizational skills
• Home health experience preferred, but not required
• Experience with WellSky or another home health EMR is a plus
