Schedule: Full time Day shift

Possible Service Locations: Hesperia CA

Pay Structure: Pay Per Visit

Job Responsibilities:

• Develop, coordinate, and maintain therapeutic services and activities in collaboration with an interdisciplinary team to help patients maintain or improve independence.
• Provide care in patients' homes, creating a comfortable and supportive environment.

Minimum Qualifications:

• Master's degree in physical therapy or an approved Physical Therapy degree per Medicare guidelines
• California Physical Therapist License upon hire
• National Provider BLS certification from American Heart Association within 30 days of hire

Preferred Qualifications:

• Experience in Home Health
• At least 1 year of experience as a Physical Therapist
