Job summary

Ascend Health is seeking a Physical Therapist to provide compassionate, patient-centered care in patients’ homes. You will assess mobility, develop individualized plans, and deliver hands-on therapy to improve strength, balance and independence.

You will educate families and caregivers, track outcomes, document progress, and coordinate with an interdisciplinary team to ensure smooth transitions between service levels.

Qualifications

• Master's degree required and current PT licensure in the state of operation.
• 1+ year of experience in home health, acute care, or rehab facility.
• Strong communication and patient education skills.

Responsibilities

• Provide compassionate, patient-centered care in the home during visits.
• Conduct in-home assessments to evaluate mobility, function, pain, and safety.
• Develop individualized treatment plans to improve ADLs, strength, balance, and independence.
• Deliver hands-on therapy and interventions in the patient’s home.
• Educate patients, families, and caregivers on exercises and safety practices.
• Track and evaluate patient outcomes and adjust plans of care accordingly.
• Collaborate with interdisciplinary team to coordinate care and transitions.
• Document visits, treatments, and progress per agency standards and regulations.
• Maintain reliable attendance to ensure continuity of care.

The Physical Therapist is responsible for, but not limited to:

• Provide compassionate, patient-centered care in the home, creating a positive and supportive experience during each visit
• Conduct in-home assessments to evaluate patients’ mobility, functional abilities, pain levels, and overall safety
• Develop individualized treatment plans aimed at improving ADLs, strength, balance, and overall independence
• Deliver hands-on therapy and interventions in the patient’s home using evidence-based techniques and appropriate assistive devices
• Educate patients, families, and caregivers on exercises, mobility techniques, and safe practices to support progress between visits
• Track and evaluate patient outcomes, adjusting plans of care as needed to ensure the most effective results
• Collaborate with interdisciplinary team members to coordinate care and support smooth transitions between levels of service
• Document all visits, treatments, and progress in compliance with agency standards and home care regulations
• Maintain reliable and timely attendance to ensure continuity of care and a strong patient-provider relationship
