Job summary

Ascend Health seeks a dedicated Physical Therapist to provide compassionate, patient-centered care in the home setting as a PRN provider in New Jersey. You will assess mobility and safety, develop individualized treatment plans, and deliver hands-on therapy to promote independence.

Qualifications

• Master's Degree in Physical Therapy
• Current Physical Therapist Certification in the state of Operations
• 1+ years experience in Home Health, Acute Care setting or Rehabilitation Facility

Responsibilities

• Provide compassionate, patient-centered care in the home, creating a positive and supportive experience during each visit
• Conduct in-home assessments to evaluate patients' mobility, functional abilities, pain levels, and overall safety
• Develop individualized treatment plans aimed at improving ADLs, strength, balance, and overall independence
• Deliver hands-on therapy and interventions in the patient's home using evidence-based techniques and appropriate assistive devices
• Educate patients, families, and caregivers on exercises, mobility techniques, and safe practices to support progress between visits
• Track and evaluate patient outcomes, adjusting plans of care as needed to ensure the most effective results
• Collaborate with interdisciplinary team members to coordinate care and support smooth transitions between levels of service
• Document all visits, treatments, and progress in compliance with agency standards and home care regulations
• Maintain reliable and timely attendance to ensure continuity of care and a strong patient-provider relationship
