Job summary

Community Medical Centers in Fresno, CA is seeking a Physical Therapist for our Home Health Rehab team. You will evaluate, plan, and implement therapy for patients in their homes, educate families, and coordinate with the care team.

Minimum requirements include a Bachelor’s Degree in Physical Therapy, California PT license or eligibility, valid California driver’s license, and current BLS for Healthcare Providers. Preferred one year of inpatient or outpatient PT experience.

Qualifications

• Bachelor's degree in Physical Therapy.
• State license as a California Physical Therapist or eligibility to obtain license.
• Current Basic Life Support (BLS) certification.
• Valid California driver's license or acceptable driving record.

Responsibilities

• Evaluating, planning, organizing, and implementing specific treatment programs for individual patients.
• Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy.
• Educates the patient and/or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information.
• Communicates effectively with other patient care team members and coordinates care provided.
• Supervises PT Assistants with implementation of treatment plan.

State of California Physical Therapist License or eligible for licensure

Valid California Driver's License and driving record acceptable to Community Medical Centers required

Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)

Preferred qualifications:

• 1 year of experience in an inpatient or outpatient clinic setting
