Job summary

Compassionate Care Home Health in California is seeking a dedicated Physical Therapist to join our patient‑centered team. You’ll assess, plan, and implement rehabilitation programs in patients’ homes, collaborating with families and caregivers to promote independence and safety.

The role emphasizes clinical excellence, clear communication, and the ability to manage multiple visits in a flexible schedule. A California PT license, CPR, and a minimum of one year of home health experience are

Qualifications

  • Master's or Doctoral degree in Physical Therapy from an accredited program.
  • Licensed to practice as a Physical Therapist in California.
  • Minimum of one year of experience; home health experience preferred.
  • Strong verbal and written communication and organizational skills.
  • Current CPR certification.

Responsibilities

  • Assess therapeutic, rehabilitative, and functional status, including home environment.
  • Assess muscle strength, mobility, gait, range of motion, and potential for rehabilitation.
  • Direct and implement physical therapy treatments.
  • Instruct patients and families/caregivers in use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report patient reactions to treatment or changes in condition to the physician.
  • Initiate physical therapy programs and educate staff as needed.
  • Train patients in the use of prosthetic devices.
  • Identify patient and caregiver needs for other home health services and make referrals as appropriate.
  • Prepare and submit clinical and progress summaries based on goal attainment.
  • Participate in patient discharge planning.
  • Provide PT consultation to families/caregivers when indicated.
  • Provide in-service education programs for nursing personnel as needed.
  • Participate in peer consultation.
  • Supervise physical therapy assistants according to policy and state regulations.
  • Instruct, supervise, and evaluate home health aide care biweekly when therapy is the only skilled service.

We are currently seeking to fill the Physical Therapist Position in MariposaCounty and All Surrounding Areas.

Physical Therapist Position
  • Compensation:
  • Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability, etc.)
  • Opportunity to accumulate overtime
Essential Job Functions / Responsibilities
  • Assessing and evaluating therapeutic, rehabilitative, and functional status, including home environment
  • Assessing muscle strength, mobility, gait, range of motion, and potential for rehabilitation
  • Directing and implementing physical therapy treatments
  • Instructing patients and families/caregivers in the use and care of therapeutic appliances
  • Determining priority needs for physical therapy

  • Initiating physical therapy programs and instructing other personnel or family/caregivers as needed
  • Training patients in the use of prosthetic devices

  • Providing physical therapy consultation to families/caregivers when indicated

  • Supervising physical therapy assistants according to organizational policy and state regulations
  • Instructing, supervising, and evaluating home health aide care every two weeks when therapy is the only skilled service
Position Qualifications
  • Degree from a master's or doctoral program in physical therapy from an accredited organization
  • Licensed to practice as a Physical Therapist in California
  • Minimum of one year of experience as a Physical Therapist; community/home health experience preferred
  • Strong verbal and written communication and organizational skills
  • Current CPR certification
  • Licensed driver with a reliable, insured vehicle in accordance with organizational requirements
