About Duke HomeCare & Hospice

Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serving as the home for the Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

Job Summary

Provide physical therapy services to patients/clients with impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes. Offer prevention and wellness services, including screenings and health promotion. The senior serves as a resource for the team and department, participating in orientation and teaching activities for students. The physical therapist works within practice guidelines as defined by North Carolina state law.

Work Performed
  1. Demonstrate sound clinical reasoning in evaluating data, diagnosing, and developing care plans.
  2. Collaborate with patients and families to create and update care plans with measurable goals and discharge plans tailored to individual needs.
  3. Understand and integrate the role of physical therapy within the patient's total needs and environment.
  4. Exhibit age-appropriate skills relevant to the patient's conditions.
  5. Follow intervention protocols and adjust plans as needed.
  6. Discharge patients when goals are met or services are no longer necessary, with follow-up as needed.
  7. Maintain competency in evaluations specific to your practice area.
  8. Meet patient care productivity standards.
  9. Work collaboratively with supervisors and peers to optimize team productivity.
  10. Utilize support personnel appropriately.
  11. Participate in patient coverage across divisions.
  12. Assess learning needs of patients and caregivers, initiating education timely.
  13. Use effective educational methods and monitor patient response, adjusting as needed.
  14. Ensure understanding of accessing further interventions.
  15. Complete timely, legible documentation meeting standards.
  16. Deliver clear oral reports.
  17. Perform data collection within time frames.
  18. Meet peer review standards.
  19. Provide clinical education for students, supervising 1-2 students for long-term rotations.
  20. Participate in outreach and research activities as requested.

Perform other related duties as assigned.

Knowledge, Skills, and Abilities

Ability to push-pull, lift-carry, and transfer patients.

Minimum Qualifications
Education

Bachelor's degree or certification in physical therapy; Master's or Clinical Doctorate preferred.

Experience

None beyond required education; more experience may be preferred.

Licenses, Certifications

Current Physical Therapist license in North Carolina.
