Job summary

CommonSpirit Health seeks a Physical Therapist to deliver compassionate, personalized home-based care, evaluating and treating injuries, disabilities, and diseases in diverse patient populations.

You will collaborate with therapists, patients, and families to achieve optimal outcomes, supervise PTAs, and document progress. A $15,000 sign-on bonus is offered to eligible new hires, with travel to patient homes as needed.

Qualifications

• Requires degree in physical therapy (baccalaureate, masters or doctorate).
• Licensed to practice physical therapy in ND.
• Two years of PT experience.
• Current CPR certification.
• Current MN driver's license.

Responsibilities

• Provide patient care including evaluation and treatment of patients in home care or referred by providers.
• Provide supervision for Physical Therapist Assistants and support personnel.
• Complete documentation related to patient care in a timely manner.
• Participate in staff in-services and education for providers.
• Collaborate with healthcare team and provide professional recommendations regarding patient care.
• Engage in care coordination activities (care conferences, etc.).

Job Summary and Responsibilities

As our Physical Therapist, you will provide compassionate, personalized care in the home. Our skilled staff improve lives by providing care for our patients wherever they call home. We are focused on caring for patients recovering from injuries, living with a chronic illness, and other medical needs.

Every day, you will evaluate and treat disabilities, injuries and disease using a range of manual therapy, modalities and exercises as well as providing education and consulting services. The Physical Therapist complies with all regulatory, professional, ethical, quality improvement and facility standards, performs charting and patient progress notes as well as supervision, training and assistance to department personnel. The Physical Therapist serves in a multidisciplinary capacity providing services to a wide variety of populations which may include pediatric, medical/surgical, orthopedics, outpatient. Homecare services are also provided and may increase travel frequency.

You should be personable, positive, and motivated to provide quality care to a wide variety of patients every day.

We are offering a $15,000 Sign-On Bonus to new hires who meet the eligibility requirements.

Job Requirements

Required

• Possesses a degree from a baccalaureate, Masters or Doctorate program in physical therapy approved by an accredited organization

• Licensed to practice as physical therapist within the State of ND

• Two years of appropriate experience as a physical therapist

• Possesses and maintains current CPR certification

• Current MN Driver's license

Preferred

• Community/home health experience

Pay Range

$23.05 - $32.56 /hour
