Job summary

CommonSpirit Health is seeking a Physical Therapist Assistant to deliver compassionate, patient‑centered home health care in North Dakota. You will work under the Physical Therapist, following the physician's plan of care, and collaborate with families to achieve optimal outcomes.

You will document notes promptly, participate in team meetings, and guide patients in mobility aids and prosthetic devices, ensuring high standards of care across diverse patients in the home setting.

Qualifications

• Graduate of an accredited physical therapist assistant program.
• Licensed to practice as a Physical Therapist Assistant in North Dakota.
• CPR certification current.
• Motor vehicle license: Minnesota Drivers License.
• Auto Insurance.

Responsibilities

• Provide direct physical therapy under the direction of the Physical Therapist and according to the physician's plan of care.
• Collaborate with therapy staff, patients, and families for optimal outcomes.
• Document clinical notes and updates within 24 hours of care.
• Instruct patients in use of mobility aids and prosthetic/orthotic devices.
• Maintain patient records and participate in Home Care team meetings.

Job Summary and Responsibilities

As our Physical Therapist Assistant, you will provide compassionate, personalized care in the home, under the direction of the Physical Therapist and according to the physician's plan of care. Our skilled staff improve lives by providing care for our patients wherever they call home. We are focused on caring for patients recovering from injuries, living with a chronic illness, and other medical needs.

Every day, you will provide direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.

To thrive as a physical therapist assistant, you must be a team player. You’ll constantly collaborate with other therapy staff, patients, and their families to seek the best possible treatment outcome. You should be personable, positive, and motivated to provide quality care to a wide variety of patients every day.

• Treats patient to relieve pain, develop or restore function, and maintain maximum performance
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training
• Observes and reports to the physical therapist the patient’s reaction to treatment and any changes in patient’s condition
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record within 24 hours of patient care
• Participates in Home Care team meetings and in-service Education Program

Job Requirements

Required

• Graduate of an accredited physical therapist assistant program with either an Associate's degree in science or applied science that is approved by the Amerian Physical Therapy Association
• Licensed to practice as a Physical Therapist Assistant within the State of ND
• Possesses and maintains current CPR certification
• Current MN Driver’s license
• Auto Insurance

Pay Range

$23.05 - $32.56 /hour
