Job summary

Compass Healthcare Consulting & Placement is seeking a licensed Physical Therapist (PT) for a full-time Home Health role serving Westchester County and the Bronx, NY. You will conduct 30 patient visits per week, assess patients, and implement plans of care in the home setting. All documentation is computerized.

Qualified candidates must have NYS license, a reliable vehicle, and be willing to travel within assigned counties. CHHA experience is preferred but not required.

Qualifications

• Valid Physical Therapist license required (NY).
• Minimum 1 year of post-graduate work experience.
• Must have a reliable vehicle and be willing to drive.

Responsibilities

• Visiting and providing physical therapy home health care services for patients in the community.
• Full-time PT role makes 30 visits per week.
• Assessing patients and creating and implementing plan of care for Home Care patients.
