Job summary

Main Line Health Home Care is seeking a Physical Therapist to deliver in-home therapy, perform assessments, and develop comprehensive care plans for patients in their own homes.

You will implement skilled PT treatments, educate patients and families, and collaborate with physicians to ensure optimal recovery and mobility in a home setting.

Qualifications

• 1 year clinical experience or clinical affiliation in Home Care.
• Graduate of a Physical Therapy program is required.
• Current Pennsylvania Physical Therapy license.
• Provider BLS required on hire.
• Valid driver’s license and insured automobile where applicable.

Responsibilities

• Perform detailed clinical assessments and develop a plan of care.
• Provide home-based physical therapy and implement treatment plans.
• Educate patients and families and coordinate with physicians and caregivers.
