We are looking for candidates who can help clients primarily in Philadelphia and Montgomery county area. We also cover Delaware, Chester, Lehigh and Bucks county.

Overview

As a Physical Therapist, you will provide high-quality care to patients in the comfort of their own homes. You will play a crucial role in helping patients regain their mobility and independence, improve their quality of life, and promote overall well-being. Strong clinical skills, computer skills, and PT experience in Home Health Care setting is preferred.

Responsibilities

• Educate patients and caregivers on therapeutic exercises, equipment, and preventive measures.
• Maintain comprehensive patient records, track progress, and adjust treatments accordingly.
• Complete patient notes and communication within 24 hours.

Qualifications

• Able to travel with reliable transportation as a licensed driver.

• Partner with healthcare clinical team, from doctors to home health nurses to ensure seamless coordination of care.
