Responsibilities

The Home Health Physical Therapist (PT, DPT, RPT) is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist works to determine a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates the patient's progress.

Education and Experience

Education Requirements

  • Recent graduates applying for the registration exam may be considered for provisional licensure.

License Requirements

  • Must be currently licensed to practice physical therapy in the state of practice.
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Skill Requirements

  • Membership or certification in a relevant national association (if applicable)
  • Willingness to accept management duties as applicable
  • This position can be employed directly by the Agency or contracted
