Position Overview

As a Physical Therapist, you will evaluate and provide therapy services in clients' homes within North Ozaukee County and Washington County. You will work as part of an interdisciplinary team under physician guidance to restore function and prevent disability following disease, injury, or loss of body parts.

Responsibilities

1. Assess clients' physical, cognitive, behavioral, communication, emotional, and social status; evaluate home safety, medication management, and fall risk.
2. Determine clients' functional rehabilitation status, including self-care, independence, and quality of life, per JCAHO standards.
3. Complete and submit accurate assessments, OASIS data, physician orders, and certifications promptly.
4. Design and modify interventions suited to the client's environment based on identified needs and goals.
5. Develop a care plan reflecting current research to meet client needs.
6. Participate in regular case conferences, update care plans, and coordinate with team members.

Qualifications

• Current WI PT license

• Valid WI driver's license, reliable vehicle, and insurance

Position Details

Part-time (32 hours/week), first shift, Monday to Friday—no weekends, no evenings, no on-call, no holidays. Up to a $10,000 sign-on bonus!
