Position Overview

As a Physical Therapist, you will provide evaluation and therapy services in clients' homes within North Ozaukee County and Washington County. You will work with an interdisciplinary team under physician guidance to restore function and prevent disability following disease, injury, or loss of a body part.

Key Responsibilities

1. Assess clients' physical, cognitive, behavioral, communication, emotional, and social status, including home safety, medication management, and fall risk.
2. Determine clients' functional rehabilitation status to evaluate their level of independence and quality of life, in accordance with JCAHO Standards.
3. Complete and submit accurate assessments, OASIS data, physician orders, and certifications promptly and according to policy.
4. Design and adjust interventions based on client needs, diagnoses, and research-supported practices.
5. Develop a care plan with appropriate interventions to meet client goals.
6. Participate in regular case conferences, creating and updating care plans as needed.

• Valid WI driver’s license, reliable, insured vehicle

Details

Part-time position (32 hours/week), first shift, Monday-Friday. No weekends, evenings, or holidays. Up to a $10,000 sign-on bonus!

This is an exempt position, paid per visit.
