Responsibilities:

1. Evaluate clients' physical, cognitive, behavioral, communication, emotional, and social status; assess home safety, medication management, and fall risk.
2. Assess clients' functional rehabilitation status to determine their current level of functioning, independence, and quality of life, in accordance with JCAHO Rehabilitation Standards.
3. Submit accurate and timely assessments, OASIS data sets, physician orders, and certifications/re-certification paperwork per agency policy.
4. Apply and modify skilled interventions based on the client's environment, diagnoses, prognosis, goals, and preferences.
5. Establish a plan of care supported by current research findings to meet clients' needs.
6. Conduct regular case conferences with team members, including creating and updating PCW/HHA Care Plans in line with patient needs and agency policy.

• Valid WI driver’s license and reliable, insured vehicle

Work Schedule:

First shift, Monday-Friday—no weekends, no evenings, no on-call. Includes holiday rotation. $5,000 sign-on bonus!
