Overview
The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.

Certifications:
Proof of Auto Insurance;
Basic Life Support (BLS) within 30 days - American Heart Association (AHA)American Heart Association (AHA);
Driver's License - Secretary of State (SOS)Secretary of State (SOS);
Licensed Physical Therapist (PT) within 90 days - Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR)
Education: Bachelor's Degree: Physical Therapy (Required)
